Consent to the processing of personal data

The User — an individual who has previously fully accepted the terms of the Terms of Use of Fonamed LLP, posted at sluh.kz/polzovatelskoe-soglashenie — hereby gives their Consent to the processing of personal data (hereinafter, the "Consent") to the Operator — Fonamed LLP, BIN 061140012525 — for the processing of their personal data in accordance with Article 8 of the Law of the Republic of Kazakhstan dated May 21, 2013 No. 94-V "On Personal Data and Their Protection".

This Consent is informed, conscious and aimed at achieving a specific purpose — for the User to undergo a hearing test using the eScreener software, owned by Sonova AG, Switzerland, registration number CHE-101.989.881. After the hearing test, the results are sent to the Operator and remain in anonymized form with Sonova AG, Switzerland, registration number CHE-101.989.881.

The User is informed that this hearing test is not a medical service, does not replace professional medical advice, and cannot be used as the basis for any decisions about the health of the User or other persons.

Processing of personal data

To the extent necessary to achieve the purpose of this Consent, the User, as the subject of personal data:

• gives their consent to the transfer of their personal data to third parties;
• gives their consent to the cross-border transfer of their personal data during its processing;
• gives their consent to the collection of data related to the subject of personal data and concerning their health.

List of actions with personal data for which the User's consent is given: all actions with personal data, including collection, recording, systematization, accumulation, storage, clarification (updating, modification), retrieval, use, transfer (distribution, provision, access), anonymization, blocking, deletion and destruction of personal data.

This Consent is given until it is revoked.

The User agrees that anonymized personal data may be stored by the personal data Operator, as well as by third parties involved in the processing of personal data, including after this Consent is revoked.

Purposes of data use

The User gives their consent to the processing of their personal data, including for the purposes of promoting (advertising) goods, work and services. The User agrees to receive postal mailings, email newsletters, or messages and notifications, including SMS messages, voice messages and calls to their mobile phone number and other devices, advertising and other information about services, and other advertising and information, including directly from advertisers.

This Consent is equivalent to the User's own handwritten signature on the Consent to the processing of personal data.
